This surgical hip treatment is used for treating hip dysplasia as a result of an acetabulum deformity. Acetabulum is the pelvis’s curved part that forms the hip joint socket. People with this form of hip dysplasia exhibit symptoms from birth. With that said, it’s only in adolescence and adulthood when the symptoms become more noticeable.

Typical symptoms include limited movement in the area and pain in the hip. If left untreated, this can cause painful osteoarthritis. Southern California Hip Institute (SCHI), led by committed board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding locations.

Causes of Hip Dysplasia of the Acetabulum

The acetabulum doesn’t develop fully in some people. This causes a fit that is misaligned in the socket or too shallow. The acetabulum cannot properly support the femoral head. Misalignment places increasing strain on the labrum soft tissue and cartilage lining the hip socket. This causes wear and tear and further degeneration. Hip dysplasia is a leading cause of arthritis and hip pain in men and women under the age of 50 years.

Benefits of PAO Surgery

Periacetabular osteotomy (PAO) hip preservation surgery allows for the following in patients with hip dysplasia that hasn’t led to advanced articular cartilage damage:

  • Preserves natural hip joint
  • Pain relief
  • Enhanced movement following the treatment
  • Return to regular vigorous activities, such as sports and other activities
  • Doesn’t affect natural vaginal childbirth in women

Ideal Candidates for PAO

Typically, candidates for periacetabular osteotomy are 40 years old or younger. You should have a mild to moderate condition that cannot be treated by using other surgical and non-surgical procedures. The patient should not have arthritis or advanced dysplasia. Total hip replacement is the best option in patients older than 50 years of age. It reduces pain and restores mobility. PAO won’t provide successful results if the hip joint is completely worn.

PAO Hip Surgery Process

Your surgeon will reposition the hip socket by cutting the bone around the acetabulum. Typically, hip replacement procedure lasts for 2.5 to 3.5 hours. You would need to stay for initial recovery in the hospital for 3 to 5 days. You may be given compression socks for preventing blood clots during the procedure. The surgery is performed under general anesthesia in most cases.

An incision of about 6 to 7 inches in length will be made over the hip crest to allow surgeons access to the pelvic bone. Three bones surrounding the acetabulum are cut using special cutting tools. Thereafter, the surgeon will shift and reorient the acetabulum to ensure the femoral head has a proper fit. About 2 to 3 long screws may be placed firmly for reattaching the acetabulum to the pelvis. This will ensure that everything stays in place.

You should be able to walk using a walker or crutches after 4 – 5 days of the surgery. Bruising, numbness, tingling, and popping sound should be expected following the procedure. Dr Garabekyan 264x300 - How To Pick Your Hip Surgeon?Choosing the right surgeon is vital when planning a hip replacement procedure. In fact, this is probably the most crucial decision in the entire procedure. The surgeon’s training and experience will have a major impact on your recovery and the results.

Southern California Hip Institute (SCHI), led by seasoned board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding communities.

Here are a few necessary aspects to consider while picking an orthopedic surgeon for hip replacement:

Verify Surgeon’s Credentials

There are certain ways to verify the surgeon’s credentials before you make an appointment. Most of these details are usually available online.

Medical licensing

Every medical doctor has to be certified by their state’s medical board. Certain states have websites that list individual doctor’s credentials. You can check the credentials of your surgeon on Federation of State Medical Boards website DocInfo.org as well.

Fellowship training

Surgeons with fellowship training in hip replacement have learned the right technique of performing the surgery under highly experienced orthopedic surgeons. These kinds of fellowships will say orthopedic reconstructions, total joint reconstruction, hip surgery, hip replacement, or something similar in their names.

Professional affiliations

Typically, qualified hip replacement surgeons are members of professional organizations, such as the American Association of Hip and Knee Surgeons and American Academy of Orthopedic Surgeons.

Prioritize Experience Over Cost

Choosing an experienced surgeon has a major impact on the success of a hip replacement surgery. A study examined data from 20,000 surgical procedures to observe that surgeons with experience provided results that mostly did not need any revision surgery.

  • Revision surgery is required only 0.7% times when the surgery is performed by experienced surgeon that do more than 50 hip replacements in a year
  • Revision surgery is required 1.3% times when the surgery is done by a surgeon that performs 6 – 25 hip replacement procedures in a year

Surgeons that perform more than 50 hip replacements in a year have a better chance at successful outcomes. The surgeon’s website should indicate that they specialize in hip replacement procedure. You can ask about this during the initial consultation as well.

Ask Questions

You are entitled to assess a surgeon before choosing them. Don’t refrain from asking the surgeon questions such as:

  • How many times in the past year have you performed a hip replacement surgery?
  • Can you share any testimonials from patients that underwent this surgery? Can I talk to these patients?
  • What is your success rate in hip replacement procedures?

Get a Second Opinion

This is not always necessary. Pertaining to this, you may want to seek a second opinion from another orthopedic surgeon if the first surgeon:

  • Lacks qualifications or experience
  • Doesn’t explain the procedure or recovery process in detail
  • Doesn’t answer your questions completely

shutterstock 1020732736 300x205 - How To Prepare For Hip SurgeryYour primary care physician will recommend an orthopedic surgeon if they think that hip replacement surgery is the best available option for you. The orthopedic surgeon will complete a full physical exam and evaluate your hip joint. They will order x-ray tests to develop a customized surgical plan.

Southern California Hip Institute (SCHI), led by board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding communities.

Preparing For Hip Replacement Procedure

Your plastic surgeon will schedule a series of appointments for testing and clearance as part of preparing for the surgery. Studies may be performed during these appointments, such as EKG, urine analysis, lab testing, and x-rays.

The surgeon will assess medical risk before the surgery. The healthcare provider will check your body weight and review your medical history. They will evaluate your lower extremities before the surgery. These are a few actions that will be taken by the healthcare provider:

  • Muscle testing: Strength of the hip extensors, flexors, adductors, and abductors. They will also check limb strength from below the hip.
  • Vascular status and nerve testing: Perineal and sciatic nerve function testing, assessment of arterial pulses, and sensory examination will be performed. They will check for severe vascular disease.
  • Palpation testing: Palpation helps in ruling out other causes of pain that are not emanating from the joint
  • Range of motion testing: Hip abduction/adduction, flexion/extension, and internal/external rotation
  • Trendelenburg sign: Tests the function and strength of the hip abductor muscles
  • Measurement of leg length

X-rays of the pelvis and hips will be taken for assessing the structure and status of the hip joint.

Preparation Steps

You may be required to take certain steps for ensuring a safer and easier recovery following the surgery. You should consider making a few simple changes in your space so that things are within your reach. Arrange for a friend or family to take care of you following the surgery. Cut down or quit smoking to improve recovery rate.

You may find it helpful to lose weight (if needed), get an obstructive sleep apnea screening, and resolve any other conditions you may have. Don’t eat or drink after midnight if you are getting the surgery performed early morning.

Taking Medications

You should inform your surgeon of all medications you are on, including prescription and over-the-counter medications. Certain medications are not recommended to be used with anesthesia, such as blood thinners, ibuprofen, and aspirin. You may have to stop taking medications before you can get your hip replacement

Visiting the Dentist

Consider having your teeth cleaned and cavities filled. This will ensure that you don’t need to visit a dentist in the first few months of surgery. It is better to not have any routine dental work performed for at least 3 months after a hip replacement surgery since the blood supply to hips is increased. Dental work may impact your healing.

shutterstock 1117121852 - Hip Surgery Procedure Stepsshutterstock 1409572676 300x200 - Hip Surgery Procedure StepsYour orthopedic surgeon may ask you to get a complete physical exam before the surgical procedure if you decide to have a hip replacement surgery performed. This is required for assessing your overall health and whether you can successfully complete the recovery process.

Southern California Hip Institute (SCHI), led by board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding locations.

Hip Replacement Orthopedic Evaluation

There are several components to having an evaluation with an orthopedic surgeon:

  • Medical history: Your orthopedic surgeon will gather necessary information regarding your general health. They will ask questions regarding the extent of hip pain and how it compromises your ability to perform daily activities.
  • Physical exam: This will assess hip strength, mobility, and alignment.
  • X-rays: X-ray images helps in determining the extent of deformity and damage in your hips.
  • Other tests: MRI scans and other tests may be required for determining the condition of soft tissues and bone of the hips.

Surgical Steps

You may be admitted to the hospital on the day of the surgical procedure. Or, you may be asked to go home the same day. You should discuss this with your surgeon before the operation.

Type of Anesthesia

A member of the anesthesia team will evaluate you on arrival at the surgery center. General anesthesia (you will be under sleep) and epidural, spinal, or regional nerve block anesthesia (your body is numb from waist down, but you are awake) are common types of anesthesia.

Components of Implant

There are several different types of materials and designs used in artificial hip joints. Two basic components include:

  • Socket component: This is a durable cup made of metal, ceramic, or plastic that may have an outer metal shell
  • Ball component: This is made of highly polished ceramic or strong metal

Prosthetic components are usually ‘press fit’ into the bone. This allows he bone to grow around the components. Or, it may be cemented using bone cement. Several factors determine which approach to use. Your surgeon may also use a combination of non-cemented socket and cemented stem. The orthopedic surgeon will understand your expectations and concerns to determine the best type of prosthesis that meets your needs.

Actual Surgical Procedure

The actual surgical procedure doesn’t usually take more than 1 – 2 hours. The orthopedic surgeon will remove the damaged bone and cartilage and position the new plastic, metal, or ceramic implants for restoring the function and alignment of the hips.

You will be moved into the recovery room following the surgery where you will be kept for several hours. You will be monitored while you recover from the effects of anesthesia. You will be discharged or taken to a hospital room after you wake up.

Recovery Process

The success of the hip replacement surgery will largely depend on whether you follow the orthopedic surgeon’s instructions regarding care in the initial few weeks. shutterstock 1409572676 300x200 - Hip Replacement Surgery RisksIn general, hip replacement complications are not major. However, you need to understand that this is a major surgery and involves the risk of serious, even if, rare problems. Weight, age, and health of patients are major contributing factors. One of the biggest risk factors is diabetes. Substance abuse, smoking, and certain medications can have an impact on the person’s risk of developing complications.

Southern California Hip Institute (SCHI), led by board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding locations.

Possible Complications

There are a few complications and risks associated with hip replacement surgery including cardiac and respiratory malfunction. Other complications may occur right away or after a few years, including:

  • Blood clots: About 1% of the people undergoing hip replacement may develop deep vein thrombosis (blood clot in the leg) or pulmonary embolism (clot in the lungs)
  • Infection: Infection occurs in only 0.4% to 1.5% patients
  • Loosening: Wear and tear can cause loosening of the implant. This is a long-term problem and is becoming rare as prosthetic surfaces and materials get better.
  • Breakage: Less than 0.5% patients may experience a broken implant because of wear and tear
  • Difference in length of leg: It is rare for patients to end up with one leg longer than the other requiring a shoe lift. A skilled surgeon will correctly measure the length of leg during and after the procedure.
  • Instability or stiffness of the joint: Heterotopic ossification may cause the joint to stiffen
  • Fracture
  • Injury to blood vessels and nerves
  • Weakness
  • Need for additional surgery

People with systemic lupus or severe rheumatoid arthritis are at an increased risk for complications. Patients suffering from malnourishment, obesity, diabetes, and hemophilia are also at a higher risk of complications. You need to make sure that you speak with an orthopedic surgeon immediately.

Dislocation

Dislocation occurs when the ball portion of the hip implant slips out of its socket. This is a rare complication and usually occurs when tissue around the artificial hip is still in the healing phase. Dislocation doesn’t usually require revision surgery. It can be repaired by popping the ball portion back into place.

Effect on Bladder Control

Hip replacement surgery should not have any affect on your bladder movement and control. You should speak with your healthcare provider if you experience any new symptoms following the procedure.

Playing Sports

Running and other high-impact sports are usually not recommended. You may resume cycling, walking, golf, and swimming among other low-impact sports once you recover from the surgery (with permission from the healthcare provider.) Don’t forget to discuss all your desired and expected activities with the surgeon.

Necessity for Hip Revision Surgery

Joint replacements don’t always last forever. However, there has been an increase in the lifetime of modern prostheses as compared to earlier prostheses. You may need a hip revision surgery if the artificial hip becomes painful.

back pain 300x205 - Questions To Ask Your Orthopedic Surgeon Before Hip Replacement SurgeryYou need to choose the right surgeon to get successful results from your hip replacement surgery. Your surgeon will recommend where the incision needs to be located, size of the incision, and the type of artificial implant to be used. The surgeon will also play a major role in factors affecting your recovery.

During your initial consultation, it is prudent to ask questions from your prospective hip replacement surgeon to clear your doubts and concerns. Southern California Hip Institute (SCHI), led by board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding communities.

How many years of experience do you have?

You should not hesitate to ask for specific information about the orthopedic surgeon’s skills and experience. This includes the number of times they perform the specific surgical technique recommended for you. You should also ask about the complication and success rates. According to research, surgeons that perform frequent hip replacements in a year have lower complications rate.

Where will you place the surgical incision?

Generally, posterior approach that involves making incisions at the back of the hip is used in traditional hip replacement. Lateral approach (side of the hip) and anterior approach (front of the hip) are other possible incision sites.

What should my expectations be from the procedure?

You should discuss specific expectations and goals with the surgeon. For instance, you should ask whether the surgery will:

  • Eliminate a limp
  • Enable you to cut your toenails or put on socks
  • Remove the need for pain medications
  • Allow you to play sports
  • Make sex comfortable

Depending on your overall health and anatomy, expected results can differ between people.

What type of implants are suitable for me?

The ball-and-socket components can be of plastic(polyethylene), metal, ceramic or a combination of all these materials. Certain components may be attached with help of bone cement. “Cementless” prosthetics are available too that allows bone tissue to grow within it over time.

There are benefits and risks associated with different prosthetics. Currently, definitive scientific evidence that shows certain implants to be better than others is not available. Your surgeon will make recommendations based on your unique situation and their experience.

Does the procedure have any possible complications?

Hip replacement procedure is a major surgery. It carried potential risks, like deep vein thrombosis (DVT) and infection. Serious complications may not be that common, but can be life-threatening. You should understand where you stand in terms of possible complications and whether any prevailing condition (like diabetes) may add to your risk.

Will you be clearing my doubts before and after the procedure?

Your surgeon should be available and approachable to answer your questions in a clear, concise, and honest manner. You will be given detailed online or written material to understand the procedure better. You should also know the number to call if you have any concerns or questions between appointments.

What is involved in the physical therapy?

Physical therapy is essential to recovery and will involve stretches and strengthening exercises. You will need to continue with physical therapy exercises at home too. Board certified orthopedic surgeon Dr. Tigran Garabekyan of the Southern California Hip Institute (SCHI) receives patients from Los Angeles, Century City, Beverly Hills, CA, and nearby areas for orthopedic surgery.

shutterstock 322577237 300x200 - Hip ReplacementYour healthcare provider may recommend getting a hip replacement if you are experiencing tremendous pain in the hip. The surgery replaces the existing hip bone with an artificial one to enable you to move without pain.

Southern California Hip Institute (SCHI), led by board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding locations.

Why Do You Need Hip Replacement?

You may require hip replacement in the following instances:

  • Osteoarthritis
  • Femoroacetabular impingement syndrome
  • Inflammatory arthritis (psoriatic arthritis, rheumatoid arthritis)
  • Developmental hip dysplasia
  • Injuries, like hip joint dislocation and hip fracture because of a fall
  • Osteonecrosis (avascular necrosis)
  • Injury that did not heal in the right manner
  • Neoplasms
  • Slipped capital femoral epiphysis and Legg-Calvé-Perthes disease among other childhood hip disorders

Symptoms Indicating Hip Replacement

  • Pain in the trochanteric and buttock region
  • Pain while at rest and performing activity
  • Pain in the groin and anterior hip
  • Pain that gets worse with putting weight on the leg
  • Loss of motion
  • Tightness and stiffness of the hip
  • Difficulty sleeping
  • Difficulty putting on socks and shoes
  • Difficulty walking

Types of Hip Replacement

Two major hip replacement types are:

  • Total hip replacement: This is the most common type which involves replacing both acetabulum and femoral head with a prosthesis.
  • Partial hip replacement: Femoral head is substituted in this procedure. It is usually performed on patients with specific hip fractures.

Overview of Minimally Invasive Hip Replacement

Minimally invasive hip replacement is aimed at minimizing the impact of surgery on muscles, blood vessels, and other healthy tissue. Anterior hip replacement has usually been marketed as a minimally invasive surgery. Nowadays, orthopedic surgeons combine surgical approach with minimally invasive technique for accessing the hip.

The surgeon will access the hip joint by moving the muscles aside by making one or two small incisions in a minimally invasive procedure. There are certain advantages to this approach, such as:

  • Less pain
  • Lower risk of muscle damage
  • Less limping
  • Easier and quicker recovery
  • Lower chance of hip dislocation
  • Shorter hospital stay

Minimally invasive hip replacement surgery is not appropriate for all types of patients. Your weight, fitness level, age, and other factors will help determine whether you are an ideal candidate.

Traditional Hip Replacement

Traditional hip replacement procedure involves one large incision which allows the surgeon to gain access to the hip. This is usually from the back (posterior approach) or through the side (lateral approach).

Recovering from a traditional hip replacement may take time since the surgeon will need to detach or cut through some muscles and tendons to get to the joint. The tendons and muscles repair as the hip implant settles in place. You may be at risk for a hip dislocation until the supportive structures get healed.

Key Takeaways

Several factors determine the surgical approach your doctor will take. This includes the style and type of implant, how the surgeon will gain access to the hip, your age, activity level, how the implant will be attached, and the health and shape of the hip bones.

AdobeStock 66719504 1 300x200 - How Long Does It Take To Walk Normally After Hip Surgery?Hip replacement surgery involves the use of a manmade implant (prosthesis) for fully or partially replacing the hip. It is generally performed on adults when other treatments, such as weight loss, medicines, physical therapy, and using walker or cane have failed to help.

Southern California Hip Institute (SCHI), led by board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding locations.

When Will You be able to Walk Normally?

Physical therapy and rehabilitation are immediately started following the surgery. They will continue while you are hospitalized and at home for at least a year following surgery. Your physical therapy will monitor flexibility and strength in your hip and leg. They will also monitor your ability to sit and stand. Moreover, physical therapists provide instructions and goals to complete while at home and hospital.

You will be asked to perform daily exercises that stretch and strengthen the muscles around your hip joint. This will likely be for 20 – 30 minutes, two or three times a day. You will slowly regain the strength to climb stairs, bend and walk as you regain strength and motion. It may take several weeks to months before you can get back to normal.

Walking Safely Following Hip Replacement Surgery

You may need to use crutches or walker which your physical or occupational therapist will provide. Make sure you use these aiding devices until the therapist advises you otherwise. Your treatment provider will decide the amount of weight you can put on the surgery leg. You may be able to place 100% weight in most cases.

Your physical therapist will also help you correctly learn to use crutches and walker. You should consider attaching a bag or basket to the walker for carrying small items. Keep in mind that walking will take a lot of effort following the surgery. Your recovery is dependent on your dedication to exercises and therapy following the surgery.

Driving After Surgery

You may not be allowed to drive for a few weeks following the surgery. You should be able to resume driving once you have control of your legs and good overall strength. You should be off pain medications and be comfortable while sitting in a car. You should ask your surgeon when you can resume driving.

Climbing Stairs Following Hip Replacement Surgery

Your physical therapist will tell you the correct way of climbing up and going down stair using crutches or handrails. You would need to use your good leg, followed by crutches and then the surgery leg. When going down the surgery leg and crutches go first. This will be followed by your good leg.

Safely Reaching for Objects

Reaching requires bending and is not usually comfortable for people. You should use long-handled reachers in the first 4 – 6 weeks to get stuff. You can ask a nearby medical supply store or your occupational therapist for reachers.

shutterstock 1302552925 300x200 - What To Expect During Your Hip Surgery Consultation?Hip surgery consultation involves a comprehensive and thorough examination and evaluation. Your orthopedic surgeon, along with their team, will obtain your personal information and medical history. An initial consultation is an integral part of the surgery process.

Your surgeon will discuss certain aspects during the consultation that will play a major role in hip joint care. They may provide you with patient forms to complete before the initial visit for facilitating your experience.

The goals of a responsible orthopedic surgeon during the visit are to:

  •  Decrease pain
  • Correctly diagnose the problem
  • Identify various tests required for diagnosing the problem
  • Helping you get back to your desired activities

Southern California Hip Institute (SCHI), led by board certified orthopedic surgeon Dr. Tigran Garabekyan, provides orthopedic surgery to patients in Los Angeles, Century City, Beverly Hills, CA, and surrounding locations.

Clinical Evaluation

The surgeon will begin the clinical evaluation by discussing the history of your problem, including various signs and symptoms. A physical examination will be performed next for assessing the cause and source of symptoms. X-rays may be required for assessing the hip joint. You may be asked to bring the x-rays during the initial visit.

Other diagnostic tests, such as MRIs can be performed at the hospital or a convenient location. The goal of the initial consultation is to ascertain the cause of joint pain and to develop a customized plan for achieving a comfortable level of health and healing that allows you to live the active lifestyle you desire.

Most patient appointments usually take 1 to 2 hours for gathering medical history, performing the examination, obtaining x-rays, and completing the consultation.

The following tests may be required for a detailed consultation:

  • Consultation with a cardiologist
  • Blood test
  • Infection tests
  • Urine tests

Things You Should Consider

It is recommended that you wear comfortable and loose clothing to the consultation, especially shorts or pants that will make it easier to examine your hips. Don’t forget to bring prior MRI imaging scans and x-rays with the typed radiologist report.

Similarly, bring the Operative Note if you have had a hip surgery before. You should bring the Implant Sticker sheet as well if you have had a joint replacement. This can be obtained from the surgeon that performed the surgery.

Insurance Assistance

You should bring the health insurance card to the consultation for insurance information. Your surgeon’s team will try and work with the insurance carrier to make the process easy.

Anesthetist Consultation

You should bring your current treatment prescriptions and all your test results with you. It’s essential to screen for any focus of infections, such as urinary tract infection, dental infection, or sinusitis. In certain cases, your implant may have microbial growth and result in an infection.

Risk Factors Screening

Screening for risk factors can prevent post-operative complications. Subsequent loosening or wear of the implant is crucial too. Common risk factors regarding hip surgery are obesity, diabetes, and complex regional pain syndrome (CRPS).
